How to import Forms into Open Dental / Practice Web from a Computer
- Download your E-mailed Forms to a known location on your Computer. For this example we will be using the Downloads folder.

Now you will need to Navigate to the "SheetImages" folder on your server. To locate this folder in Open Dental / Practice Web go to, "Setup" and then "Data Paths"

Once open, right-click and copy the file path then click "Cancel" to close the window.

Open your Documents folder.

Once your Documents folder is opened, paste the file-path you just copied into the Title Bar and click the Blue arrow.

Now Locate the "SheetImages" folder and double-click into it.

Now navigate to where you downloaded your converted forms and copy and paste the image(s) into the "SheetImages" folder.

After you have copied the images go to Open Dental / Practice Web and select "Setup" and select "Sheets"

Now select "Tools"

Now select "Import"

Now you will need to navigate to where you saved your forms and select the ..XML file and select "Open" (You can only import one .XML file at a time)

Now click "OK" and select "Close"

Your Form has now been imported.
- To import another form, repeat steps 9 - 12.
